Wet Wrap Dressings for Severe Eczema

Wet wrap dressings reduce itching and inflammation by cooling the skin and improving penetration of topical medications. They also act as a protective barrier from the trauma associated with scratching.

1. Fill a basin or sink with warm tap water.

2. Soak the dressings (Kerlix, gauze, or long pajamas). Squeeze out excess water. Dressings should be wet, not dripping.

3. The child should receive a bath in warm water (lasting 15-20 minutes) immediately before this procedure. Pat skin dry with a towel.

4. Apply the appropriate topical medications to affected areas and moisturizer to nonaffected areas immediately after soaking.

5. Cover an area with wraps using two to four layers of wet gauze or other material. Immediately after wrapping, cover with appropriate dry material such as an ACE bandage, socks, or pajamas. Start at the feet and move upward. Use wet, long underwear or wet pajamas covered by dry pajamas or sweat suit with total-body involvement in place of wet gauze.

6. The skin is to remain in a moist environment. Use spray bottle or damp towel to remoisten, if necessary, after removing dry layer. If patient is up and about with wet dressings in place, watch carefully for chilling. The patient may sleep in dressings or wet clothing overnight if they tolerate it.

After all dressings are removed, moisturizers may be applied to the entire body.
